It’s shorter! This year’s Patient Safety Culture Survey is down to 23 questions.

It’s faster! It can be completed in just a few minutes.

It’s anonymous! We will never be able to track answers back to an individual.

It matters! We need as many of you to respond as possible. Your thoughts and opinions will help us decide where to focus our patient safety efforts moving forward.

Starting the week of March 21, people working in direct clinical care or clinical support roles may receive an email from “Leger Metrics” that contains a link to the 2016 Patient Safety Culture Survey.   

This link is being sent to the inbox of your Providence email account (or, for physicians, your preferred email address).

If it has been a while since you last checked your PHC email account, or if you didn’t even know you had one, you may need some help getting in! The Help Desk is on standby; please call them at 69333.

If you have checked your email and you haven’t received an email from Leger, we may not have had an email address for you in our files.

Not to worry. If that is the case and you're in a clinical role you, Leger will mail you a paper copy of the survey. You can either complete the paper version or access the survey on-line using a URL provided to you in the paper copy.
